Steps to access your recent Eye Exam data, including signed eyeglass prescription, through our Patient Portal.

Enter the email address you used to create your intake when you went to your appointment.

Enter the Patient’s Legal First Name and Date of Birth.

An email will be sent to you with a password to the portal.

In the email there will be a link to the portal with the password.

Login to the portal and enter the password.

Select the date of your eye exam that you want data from.
Please make sure you use the full name that was entered on the intake.
For example, if you entered “William” on your intake, please use “William” instead of “Will”.
If you did not provide your email, you will need to call your exam location and provide one so they can update your account to get access to the portal.
Your Exam Summary will then pop up.
For your eyeglass or contact lens prescription, scroll to the bottom.
